Eligibility :
The Promise Your Heart Contest is open only to legal residents of the 49 United States and District of Columbia, EXCLUDING RESIDENTS OF ARIZONA, GUAM, PUERTO RICO AND ALL OTHER U.S. TERRITORIES AND POSSESSIONS, PROVINCE OF QUEBEC AND ALL OTHER COUNTRIES, OR TERRITORIES, who are 18 years of age or older and the age of majority in his/her state, province, or territory of residence at the time of entry.
Timing :
The Promise Your Heart Contest (the “Contest”) starts at 12:00AM Eastern Time (“ET”) on 12/30/08 and ends at 12:00 AM ET on 3/2/09 (the “Promotion Period”).
How to enter:
To enter, visit www.prevention.com/promiseyourheart (the “Web Site”) during the Promotion Period and completely fill out all the required fields on the entry form, including first and last name, complete home mailing address (no P.O. Boxes), daytime telephone number (including area code), date of birth and e-mail address, and a 25 words of less “heart promise” and a picture of entrant in red. (the “Entry Material”). Then submit your entry form and the Entry Material to the Web Site (the “Entry” or “Entries”)
Limit:
Limit one (1) Entry per person per e-mail address per day throughout the Promotion Period.
Prize (s):
The approximate retail value (the “ARV”) of the prize is $4,000 and consists of coach airfare for two (2) and double occupancy hotel accommodations for , one day of pampering at a NYC spa of Sponsor’s choosing, hair and make up styling for one night out , tickets for one night at Broadway show of Sponsor’s choosing, and one dinner at a world-renowned New York restaurant. The total ARV of all prizes awarded is approximately $_4,000.
