How To Enter : During the Promotion Period, visit http://vsp.promo.eprize.com/envision2012 and follow the links and instructions to complete and submit the registration form, including a valid home address.
Eligibility : The 2012 VSP EnVision Eye on the Prize Sweeps (the “Sweepstakes”) is open only to legal residents of the fifty (50) United States and the District of Columbia who are at least eighteen (18) years old at the time of entry (“entrant”).
Timing : The Sweepstakes begins on February 14, 2012 at 12:00 a.m. Pacific Time (“PT”) and automatically ends on January 31, 2013 at 11:59 p.m. PT (the “Promotion Period”)
Limit : Each participant may enter only one (1) time per each Entry Period during the Promotion Period.
