Eligibility : The Sweepstakes is intended for individuals with diabetes or a family history of diabetes. To enter Glucerna’s “Life in Balance Sweepstakes” (“Sweepstakes”), an individual must be a legal resident of the 50 United States or District of Columbia be at least 18 years of age or the age of majority in his/her state of residence, whichever is greater.
Timing : The Glucerna® “Life in Balance Sweepstakes” starts 7/12/10 at 12:00 A.M., ET and ends 10/31/10 at 11:59:59 P.M., ET. (“Sweepstakes”).
Limit : One entry per day per person regardless of the method or category of entry

(13): One Other Source Entry Winner and Twelve (12) Source Entry Winners (collectively, “Winners”) will each receive as the Prize a Glucerna Life in Balance Six Month Package consisting of 31 sessions with a Fitness/Personal trainer, 1 monthly session with a Nutritionist/Dietician; 3 sessions with a Clinical Diabetes Educator; and 1 monthly session with a Life Coach who will be leading and coordinating the aforementioned professional team of experts and homework assignments/follow-up calls; and 5 additional hours for email and/or telephone consultations with either the Nutritionist, Dietician or Clinical Diabetes Educator .
